Semaglutide is the active ingredient behind two of the most-discussed weight-loss drugs in the world today — Ozempic and Wegovy. It's also available as Rybelsus, an oral tablet. What semaglutide is

Semaglutide is a GLP-1 receptor agonist. GLP-1 (glucagon-like peptide-1) is a gut hormone your body releases after meals. It does three things:

  1. Signals satiety to the brain — the "I'm full" sensation
  2. Slows gastric emptying — food sits longer in your stomach, you feel fuller for longer

Semaglutide mimics natural GLP-1 but has been chemically modified to last about a week in the body, so you only need one dose per week (or one tablet per day, for Rybelsus).

Where semaglutide is in your weight loss

In the STEP-1 trial, adults with obesity who took Wegovy (semaglutide 2.4 mg) lost an average of 14.9% of body weight over 68 weeks, compared to 2.4% on placebo. That magnitude of medical weight loss was previously only achievable with bariatric surgery.

Translated: a 90 kg adult could expect to lose roughly 13 kg over 14 months on Wegovy, with appropriate diet and movement support.

Lower doses (Ozempic 1 – 2 mg, Rybelsus 7 – 14 mg) produce less weight loss but still meaningful improvements for patients with type 2 diabetes.

How to take it

Wegovy / Ozempic (injection):

  • The narrow ingestion window matters — Rybelsus absorption is poor if taken with food

The pattern is slow and steady, not dramatic. People who expect 10 kg in the first month are usually disappointed; people who plan for a year-long medical process do better.

Side effects: what's real and what to do

Common (≥10%) — usually settle in 4 – 8 weeks:

  • Nausea (~44% on Wegovy, less on lower doses)
  • Constipation
  • Diarrhoea
  • Mild reflux
  • Headache and fatigue early on
  • Reduced appetite (intended)

Management strategies your doctor will discuss:

  • Eat slowly, stop when satisfied (not full)
  • Smaller meals, more often
  • Higher protein, lower fat in the first weeks
  • Hydration — 2 – 2.5 L daily
  • Avoid alcohol and spicy/oily food during titration weeks
  • Anti-nausea medication if needed (your doctor decides)

Rare but serious — call your doctor immediately if you experience:

  • Severe, persistent abdominal pain (possible pancreatitis)
  • Persistent vomiting
  • Right-upper-quadrant pain (possible gallbladder issue)
  • Signs of allergic reaction (rash, breathing difficulty, swelling)
  • A lump in the neck or persistent hoarseness (rare thyroid concern)

Who can't take semaglutide

Semaglutide is not appropriate if you have:

  • Personal or family history of medullary thyroid carcinoma or MEN2 syndrome
  • History of severe pancreatitis
  • Severe gastrointestinal disease (e.g., gastroparesis)
  • Currently pregnant, breastfeeding, or planning pregnancy in the next 2 months
  • Type 1 diabetes (off-label use only)

How to access semaglutide legally in India

  1. Get a medical assessment — BMI, history, current medications, conditions.
  2. Get appropriate blood work — fasting glucose, HbA1c, lipid panel, kidney + liver function, TSH.
  3. Consult a registered Indian medical practitioner — in person or via telemedicine (Telemedicine Practice Guidelines, 2020).
  4. Receive a written prescription — required by law for purchase.

Frequently asked questions

Is semaglutide legal in India? Yes — Ozempic, Wegovy, and Rybelsus are all approved in India. They are prescription-only.

Can I take semaglutide for weight loss without diabetes? Yes — Wegovy is specifically indicated for chronic weight management in adults with BMI ≥ 30, or ≥ 27 with comorbidities. Diabetes is not a requirement.

Is the oral version (Rybelsus) as effective for weight loss? Rybelsus produces less weight loss than Wegovy because the maximum dose (14 mg/day) is lower than what's used in Wegovy (2.4 mg/week, with much higher bioavailability). It's an option for patients who can't or won't inject.

Do I take it forever? Stopping abruptly typically leads to weight regain. Most patients use semaglutide long-term, much like medications for hypertension. Duration is a shared decision with your doctor.

Can I drink alcohol on semaglutide? Light, occasional alcohol is generally tolerated once side effects have settled. Heavy alcohol use is discouraged — it worsens nausea, raises pancreatitis risk, and undermines weight loss outcomes.
