The market is full of pills, powders, and "fat burners" sold over the counter or online. Most of them are unregulated, unsafe, or both. The list of medications actually approved by the CDSCO for weight management in India is short. This guide explains what's legal, what each medication does, who it's for, and how to access them safely.

The short list of legal weight loss medications in India

Class Generic Brand(s) Typical weight loss Status
GLP-1 receptor agonist Semaglutide Ozempic, Wegovy 10 – 17% Prescription
Tirzepatide Mounjaro 15 – 22% Prescription
Oral semaglutide Rybelsus 5 – 8% Prescription
Lipase inhibitor Orlistat Xenical, Obelit 3 – 5% Prescription / OTC (varies)

Other globally-used medications (naltrexone-bupropion, phentermine) have very limited or no approved availability in India in 2026.

What's illegal or unsafe

  • Imported GLP-1 without a prescription — illegal, frequently counterfeit, often cold-chain failed
  • "Fat burner" supplements containing undeclared sibutramine, DNP, or amphetamine analogs — these have been responsible for serious adverse events
  • Compounded semaglutide from unregulated sources — purity and dose are unverified
  • Herbal or Ayurvedic preparations claiming dramatic weight loss without disclosure of active ingredients

If a product promises "5 kg loss in 2 weeks" or "doesn't require a prescription," it's either ineffective or unsafe. Both options waste your money.

GLP-1 medications: the dominant category in 2026

GLP-1 receptor agonists are the most-prescribed weight management class globally because of their outsize weight-loss outcomes and acceptable safety profile under supervision.

Semaglutide (Ozempic, Wegovy, Rybelsus): Once-weekly injection (Ozempic, Wegovy) or daily oral (Rybelsus). Mimics a gut hormone that signals fullness, slows gastric emptying, regulates blood sugar. Wegovy at 2.4 mg produces ~15% average weight loss over 68 weeks.

Tirzepatide (Mounjaro): Once-weekly injection. Dual agonist on both GIP and GLP-1 receptors. At 15 mg produces ~21% average weight loss over 72 weeks.

See our comparison guide for how a doctor picks between them.

Orlistat (Xenical, Obelit)

Orlistat works completely differently — it blocks ~30% of dietary fat from being absorbed. Average weight loss is modest (3 – 5%) and side effects (oily stools, urgency, vitamin malabsorption) are significant. It has a role for specific patient profiles but isn't the dominant choice anymore.

How a doctor decides

A real clinical decision considers:

  1. BMI + comorbidities — establishes whether medication is appropriate at all
  2. Contraindications — rule out medullary thyroid carcinoma history, MEN2, severe GI disease, pregnancy, etc.
  3. Patient preference — injectable vs oral, weekly vs daily
  4. Cost — pragmatic constraint that matters
  5. Drug interactions — especially with insulin, sulfonylureas, warfarin, or oral contraceptives
  6. Prior tolerability — if you've failed one GLP-1, another may still work

This decision happens in a 20 – 30 minute consultation, not a "fill out a form and we ship it" workflow.

The legal access path

  1. Online or in-person medical assessment
  2. Recent blood work (fasting glucose, HbA1c, lipids, kidney + liver, TSH)
  3. Video or in-person consultation with a registered Indian medical practitioner
  4. Digital prescription with the doctor's registration number
  5. Purchase from a licensed pharmacy with appropriate handling (cold chain for injections)
  6. Continuous follow-up for dose titration and monitoring

Skipping any step is unsafe and illegal.

Frequently asked questions

Can I get weight loss medication without a prescription in India? No legitimate, safe weight loss medication is available without a prescription. Orlistat is OTC in some forms but is not the first-line choice for most patients.

Are GLP-1 medications safe for long-term use? Long-term safety data is reassuring. Ongoing supervision allows your doctor to monitor for the rare serious side effects (pancreatitis, gallbladder disease, thyroid concerns).

Will weight come back if I stop the medication? Most patients regain weight after discontinuation if they stop abruptly without a transition plan. This is why discontinuation is a planned process discussed with your doctor.

What's the cheapest legal weight loss medication? Orlistat is the cheapest, but it produces less weight loss than GLP-1s and has significant tolerability issues. Rybelsus (oral semaglutide) is the cheapest GLP-1 option, at ~₹9,000 – ₹13,000 monthly.

Does insurance cover any of these? Most Indian retail policies don't. Some corporate / premium policies cover GLP-1s when prescribed for type 2 diabetes. Check your policy.

Can I get a prescription via video consultation? Yes — the Telemedicine Practice Guidelines (2020) allow it for these medications.
